NOTICE OF DISSOLUTION/CLAIMS

MDMA EQUIPMENT, LLC

Please be advised that MDMA Equipment, LLC, a Wisconsin limited liability company, dissolved effective April 2, 2012.

Any claims against the dissolved limited liability company or its members or officers shall be barred unless a proceeding to enforce said claim is brought within two years after the date of publication of this notice.

Any person or entity who has a claim against this limited liability company or its officers or members in their capacity as such, must present said claim in accordance with this Notice. Claims must be filed in writing with:

Claims must contain a description of the basis for the same, date(s) on which the claims allegedly arose, the dollar amount of such claims, and the name, address and telephone number of the claimant.
